Health Care

Preventive Health Care for the Aging (PHCA)
You have a Doctor...But do you have a Nurse?

Photo Credit given to www.gettyimages.comPreventive Health Care for the Aging (PHCA) services are now up and running at The Center at Founders Village, Senior and Community Center. PHCA is a state and county funded program that offers free health assessment for those over 55 years old. The complete assessment takes about an hour and will include a health history, 24 hour nutrition recall, pulse, blood pressure, height, and weight. No medical treatment is provided but a referral to a doctor may be given for a major health problem. A care plan is devised between the client and nurse for suggested action. Other assessments also offered are: vision, urine, hemoglobin, blood glucose screening, skin, oral/dental examination, hemoccult kit or hearing tests.

Appointments are recommended. Call 714-593-4446 for appointment. The nurse, Jane Hannah is at the center the 2nd Friday and various Tuesdays each month. Jane is a certified gerontology nurse as well as a certified diabetes educator. She has been with PHCA for over 25 years. PHCA and Talbert Medical Group also cosponsor a diabetes education and support group once a month on the 2nd Friday from 1pm-2:30pm.
